The State of New Hampshire requires documentation of immunizations on file for each child by the first day the child is in attendance. 

PLEASE NOTE: DUE TO THE SERIOUS PUBLIC HEALTH IMPLICATIONS FOR NONCOMPLIANCE WITH CDC IMMUNIZATION RECOMMENDATIONS, HAZEL DRIVE EARLY CHILDHOOD CENTER DOES NOT ACCEPT CHILDREN WHO ARE NOT IMMUNIZED EVEN WITH AN EXEMPTION FORM. 

We do not require children to be immunized for COVID-19.

No more than 60 days after registration, an up-to-date child health form, which includes any special health considerations or known allergies, must also be on file and updated annually.

Please notify us if your child is going to be absent due to illness.

A child with any of the following symptoms must stay at home until 24 hours after the symptoms have subsided.

  • Inflamed eyes
  • Ear ache
  • Rash
  • Diarrhea
  • Sore throat
  • Vomiting
  • Fever
  • Frequent cough and/or heavy nasal discharge
  • Wheezing
  • Unusual or extreme fatigue or lethargy

A child who develops any of these symptoms while at school will be given a comfortable place to rest apart from the group and a parent will be called to come get the child.  If you cannot be reached, a person listed on the child’s registration form will be called.

Administration of Medication

Prescription and non-prescription medication may be administered under the following conditions:

  • It is in its original container
  • It is labeled with the child’s name
  • A State of New Hampshire medication authorization form has been completed by the parent

You should request the form at drop-off and you’ll need the medication in hand in order to complete the form.  Medications must be given to the teacher. Please do not put medications in lunch bags or backpacks as they must be kept out of children's reach.

Any medication that has been given at home before your child comes to school should be reported in a note that includes the name of the medication, the time and amount administered, the reason it was given and any potential side effects.
